Signature Requirements for Hospital--Administered Grants

Investigators are advised to contact the hospital's research office to ascertain the requirements for processing grant applications. However, the following signatures (at least) will be required in this particular order:

• The Investigator must sign the application form as required
• The University Department Chair or his/her delegate as appropriate must sign funding applications for faculty members
• The Hospital Research Institute Director or his/her delegate must sign on behalf of administering institution

Where a signature from the Dean of Faculty is required by a funding agency, this is delegated to the hospital's Research Institute Director.

Applications may be submitted to funding agency(ies) once all authorizing signatures have been obtained.
