Medical Psychiatry Collaborative Care Certificate (MP3C)
The Medical Psychiatry Collaborative Care Certificate (MP3C) is a continuing professional development program aimed at building capacity for integrated and collaborative care in healthcare teams and individual providers. It focuses on education resources for healthcare providers to better equip them to assess and manage patients with co-existing physical and mental health needs.
MP3C curriculum is built on seven core program competencies that inform individual module objectives and learning outcomes:
- Identify and manage patients with co-occurring mental and physical health conditions using an integrated and comprehensive approach
- Describe roles, responsibilities, and accountabilities of collaborative care team members
- Develop, implement and communicate inter-professional care plans
- Advocate for integrated patient care
- Develop integrated collaborative models of care within existing networks and resources
- Reflect on patient encounters and resource limitations, and seeks relevant resources to improve patient outcomes and experiences
- Perform continuous improvement of integrated collaborative practice
